Directions to our office
We are located in the Curtis Center in Society Hill
at 601 Walnut Street, Suite L90, (Center City) Philadelphia,
PA 19106. To get there, follow these directions:
Directions from South Jersey
Take Atlantic City Expressway to Route 42 North to
the Benjamin Franklin Bridge. Once you cross the bridge,
stay in the second from the right lane. Follow to
first traffic light and make a right. Stay in the
right land and follow 6th Street until you cross over
Market Street. In between Chestnut and Walnut Streets
is Sansom Street, which is on the right. Make a right.
Our building (The Curtis Center) is on your left side
between 6th and 7th Street.
Directions from the West
Take I-76 to I-676 East. Take the Broad Street exit
and stay in the left-hand lane of the exit. Go down
the ramp to the first traffic light. Go to the next
traffic light, which is Broad Street, and take a right.
Take Broad Street around City Hall while staying on
the right hand side of the road. Take a right onto
Market Street East. Follow Market Street to 6th Street.
Take a right onto 6th Street. . In between Chestnut
and Walnut Streets is Sansom Street, which is on the
right. Make a right. Our building (The Curtis Center)
is on your left side between 6th and 7th Street.
Fees and Payment
Our office accepts e-checks, Visa,
MasterCard, American Express and Discover. Payment information
is taken at the time of appointment and processed 48
hours prior to your appoinement. We do not accept insurance
for payment; however, we will provide you with a properly
coded itemized receipt that includes the appropriate
diagnostic and billing codes. Many insurance plans will
cover part of the cost of office visits according to
their out-of-network provider schedule and may cover
some or all of the costs of diagnostic testing.

Cancellation
Policy
In our practice, we see one patient
at a time so that each patient gets our undivided
attention and we strive for 100% on-time consultations.
Accordingly, our cancellation policy is very strict
and refunds will not be provided. Cancellations
made 72 hours or more before the scheduled appointment
will be rescheduled and the paymemt method will
not be charged until 48 hours before the new appointment.
When appointments are cancelled within 72 hours,
the payment method will be charged based on the
previously scheduled appointment, and the appointment
will be rescheduled. If multiple appointments are
cancelled, Ultimate Vitality Centers may charge
a cancellation fee of $100.

Are bioidentical hormones FDA
approved?
Some standard dosages of bioidentical hormones are
FDA approved and we use these standard doses whenever
possible. However, when a dosage or strength that
the patient requires is not commercially available,
we use a compounding pharmacy to prepare an individualized
prescription. Compounding pharmacists must comply
with the regulation of the state boards of pharmacy,
and they have access to the highest grade bulk ingredients
as well as state-of-the art equipment used to accurately
prepare hormone preparations. Close monitoring of
the patient by PWC helps to promote maximum benefits
and safety.

What is Juvederm?
JUVÉDERM® Ultra and JUVÉDERM®
Ultra Plus are indicated for injection into the
mid to deep dermis for correction of moderate to
severe facial wrinkles and folds (such as nasolabial
folds). Side effects were usually mild to moderate
lasting 7 days or less and included temporary injection
site reactions like redness, pain, firmness, swelling
and bumps.
